The Family Support Process
Five Stages, No Surprises.
Step 1: Initial Consultation
A conversation, not an interview. We ask about routines, strengths, worries and hopes — and we answer every question you have about how we work.
Step 2: Assessment
We meet the person in their own environment, review current supports and identify what helps, what gets in the way and where safety needs attention.
Step 3: Service Planning
Goals are written in the person's own words wherever possible, paired with the specific strategies, schedules and staffing that will support them.
Step 4: Family Communication
You choose how you want updates — call, text or email — and how often. Anything urgent reaches you the same day.
Step 5: Ongoing Support
We review progress on a regular cadence, adjust the plan as life changes, and keep the same caregivers in place whenever possible.
